The care provided to mothers and babies who died or suffered serious harm following Maternity care at the Shrewsbury and Telford NHS Trust is currently the focus of an independent police investigation, Operation Lincoln.

An independent review of maternity services at the Trust has now concluded with the final Ockenden Review report being published on the 30^th March 2022. Operation Lincoln remains ongoing and continues to engage with families, individuals, organisations and services to identify cases which meet the threshold for a police investigation. Any families who have significant concerns regarding their experience of maternity care at the Shrewsbury and Telford NHS Trust are encouraged to contact Operation Lincoln directly with information regarding their experience of maternity services at the Trust.
